What is the typical protocal of telling the family (parents) of the death of their son or daughter in the service of their country?

Do they still send the next most senior officer that knew the individual?

From TV: the senior officer writes a letter and a military chaplain or PR-officer is sent to the family. After all, very often, the senior officer is still out on duty and stuff.
